Human Resource / Payroll Guidelines

As soon as a student has been identified for hire, Human Resources must be notified immediately.  Contact Seth Klein (x8372 or ) or Milly Lugo (x3285 or ).
Students must complete a Occupational Health Screening and New Hire paperwork PRIOR to beginning work (set up through Human Resources).
Students must attend a New Hire Orientation within two weeks of their start date.
Students are not eligible for benefits.
Students that are not compensated must still complete an Occupational Health Screening and New Hire Orientation.
Mentors of students receiving either CMRC Stipends or Katz Scholarships:
Recipients of CMRC Stipends and Katz Scholarships will receive two checks, each in the amount of $1,250.  The first check will be processed the last week of June 2007; the second check will be processed the second week of August 2007.
W-9 Form - Request for Taxpayer Identification Number and Certification:
Any student receiving lump-sum stipend payments (not hourly wages) from a Children’s Memorial fund must complete and submit a W-9 Taxpayer ID Form to Laura Kochan, Accounts Payable Manager, located in the Marcey Street building (Box 26). The federal government requires that earnings of $600.00 or more per calendar year must be claimed as income and taxes paid accordingly.  No checks for student stipends will be processed by Accounts Payable unless the W-9 form has been received.
