“Systematic Nutritional Assessment in Pediatric Practice (SNAPP)” strives to increase pediatrician counseling about two healthy behaviors:  improved diet and increased physical activity.  SNAPP is a practice systems intervention plan that contains: 1) a color-coded wall chart to rapidly assess nutritional status, 2) caregiver handouts with diet and physical activity recommendations specific to the child’s age and nutritional status, 3) improved charting methods, 4) practice-directed education about childhood nutrition, and 5) medical record reviews to assess baseline patterns and practice changes.
